FT Care Coordinator/MSW-LSW
Imua Family Services, one of Hawaii's 2012 Best Places to Work and a 2010 National Best Non- Profits, is seeking a full-time a Temporary to Permanent Care Coordinator for families and children ages birth to three who have developmental needs.

Candidate must:

1. Have a MSW Degree in Social Work, must be eligible to obtain Hawaii State License (LSW)

2. Have experience in Care Coordination or Case Management

3. Have experience in working with families and/or children

4. Valid drivers’ license, use of vehicle that is registered and safety inspected

5. Be able to visit homes and drive on Maui to meet clients

6. Be able to adjust schedule to meet the needs of clients

7. Be able to organize and prioritize work to meet strict time lines

8. Be able to facilitate teams and coordinate services to families

9. Have excellent interpersonal and written communication skills
